Required for all applicants under the age of 18 in India.

1. Download and fill in the COPPA Parental Consent Form below.
2. Print, sign alongside your parent or legal guardian, and scan the completed form.
3. Submit online (upload during application) or offline (bring to audition venue).

Applications without a valid consent form will not be processed.
